Indianapolis-based Community Health network has launched a joint venture with management company VillageMD to create a new model of care delivery that aims to improve quality of patient care and significantly reduce healthcare costs across the region. Called Primaria Health, the company partners with a growing network of both employed and independent primary care doctors providing the tools, strategies and support to manage care for chronically ill patient populations.

Could this be you?

An integral member of the Primaria Health team, the Patient Service Representative will be accountable for scheduling necessary appointments and referring patients to providers or caregivers. The ideal candidate will demonstrate the ability to work in a fast-paced environment while using excellent communication skills to serve our patient populations. He/she must be proactive with strong personal initiative as well as highly organized and detail oriented.

How You Can Make a Difference

  • Documenting patient data and information required for necessary scheduling
  • Referring patients and scheduling necessary appointments with providers or caregivers
  • Developing rapport with patients in order to communicate effectively regarding the course of their care
  • Collaborating with the multiple Primaria departments to deliver outreach services to chronic care patients as identified by Analytics, Care Navigation, and Population Health Operations

Experience To Drive Change

  • High school diploma or GED equivalent required
  • Associate’s degree or Medical Assistant Certification preferred
  • 1 year of experience in a physician office preferred
  • 3-5 years of call center previous experience preferred
  • 2 years of experience in a healthcare setting preferred
  • Prior experience using Electronic Medical Records reporting required
  • Strong communication and relationships skills required; demonstrated success partnering with physicians and care teams to support patient wellness
  • Comfort with technology including the Microsoft suite of products
