Position: Direct Support Specialist Part Time

Location: Frankford, NJ

Shift: Sun, Mon, Sat 2p-10p

Job Description:

Assists individuals with intellectual/developmental disabilities to lead a self-directed life and contribute to the community; assists with activities of daily living and encourages attitudes and behaviors that enhance community inclusion.

Essential Duties:

Responsibilities and Essential Duties including but not limited to the following as well as any other duties as assigned by supervisory staff.

Support each individual so that he/she will feel a sense of self-determination, security, and have an opportunity to have meaningful relationships with others at home and in the community.

Assist each individual with daily life activities including personal care, household chores, and shopping, recreational, and social activities to have a balanced life.

Provide companionship and emotional support to each individual.

Ensure individuals are safe, healthy, and living in a clean environment.

Report all incidents including allegations of abuse, neglect, exploitation, and serious reportable incidents to leadership immediately upon discovery.

Complete all required documents to include medication records, observation notes, progress notes, goals and outcomes, and financial transactions.

Provide ongoing care related to all medical and/or psychiatric needs, to include appointments, treatments, coordination of care, communication with the team.

Adherence to all treatment recommendations including but not limited to: diet, meal planning, adaptive equipment, behavioral interventions, etc.

Safely transport individuals to all activities, places of employment, outings and appointments.

Interact professionally with families, Division staff, doctors, and all other community outreach services utilized by the individuals and program.

Demonstrate knowledge of individuals’ goals and implement individualized methods for completion.

Attendance at all mandatory meetings and training’s offered by Capitol Care, Inc. to enhance staff performance, education, and knowledge of the job as well as to monthly supervisions with manager/shift supervisor. Seek out additional supervision and training as needed.

Demonstrate an understanding of regulatory and agency standards.

Adhere to all confidentiality policies, practices, and laws.

Administer medications upon completion of training; fill prescriptions, properly document prescriptions, administration, and any medication errors.

Practice and implement all emergency procedures including, but not limited to: fire, disaster drills, and other crisis response procedures.

Must be able to properly support and implement personal control techniques to ensure safety.

Qualifications:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Must be 18 years or older and possess a valid non-probational driver’s license.

Additional Requirements, Certificates, Licenses, Registrations

* Clearance from the Department of Human Services following a federal and state criminal background check, as well as checked against the Central Registry and Child Abuse Record Information.
* Pre-employment physical; Pre-employment tuberculosis test; Pre-employment and random drug screen as required by The State of New Jersey Department of Human Services.
* Clearance based on a 3-year and annual motor vehicle abstract assessment.
* Must be able to successfully complete required training which includes, but is not limited to:

American Red Cross Adult CPR and First Aid

Crisis Management

Medication Administration with on-site competency assessment

Preventing Abuse and Neglect with on-site competency assessment

Positive Behavior Supports (if applicable)

12 hours of professional development annually per calendar year prorated at one hour per month

Specialized Training as requiredTracking
